Appointment management

The administrator schedules each home visit and assigns the nurse who will attend, so nobody turns up twice or not at all.

Pain monitoring

Pain intensity, location and symptoms are recorded over time, so a nurse can see whether someone is improving before the next visit.

Rehabilitation monitoring

Nurses assign exercises from the shared library to the right patient and follow whether they were completed or missed.

Clinical assessment

Vital signs, complaints, disease history and treatment consent, recorded in the same order as the paper form used on visits.

Exercise library

Videos and images kept in one place, added once by the administrator and reused by every nurse.

Reminders

Medication, exercise and visit reminders that reach the patient by email and in their own Google Calendar.

Who uses it

Access is role based. People only see the records they are responsible for.

Administrator

  • Manages nurse and patient accounts

  • Ticks off exercises and medication as they go

Install it like an app

Open the site on a phone and choose Add to Home Screen. Reminders arrive by email and in Google Calendar, so patients do not have to remember to check.
